The undersigned voluntarily agrees to participate in the Day Hike at Saddle Mountain, hosted by the Sonoma Ecology Center on October 25, 2024 10:30 am-1:30 pm
The undersigned recognizes that Sonoma Ecology Center has not undertaken any duty or responsibility for his or her safety and for the safety of minors and the undersigned agrees to assume full responsibility for all risk of bodily injury, death, disability, and property damage as a result of participating in the aforementioned Hike. The undersigned recognizes that there are inherent risks associated with this hike. I am in good health, as is my child, and we are aware that this outing is for experienced hikers, with moderately strenuous hiking for four (4) miles. There is a substantial elevation gain on established trails and fire roads. We may pass through brushy areas, with possible exposure to ticks or poison oak. We will cross bodies of water. I am wearing appropriate footwear, as is my child, and I have taken other personal precautionary measures, such as wearing sunscreen, long pants, long sleeves, hat, etc. In addition, I have brought enough water for my party, and lunch. I understand there will be changes in the weather.
I (we), as parent(s) or in Loco Parentis, of the minor, do hereby for my child, myself, my heirs, executors, and administrators, release and forever discharge Sonoma Ecology Center and all officers, directors, employees, agents and volunteers or the organization, acting officially or otherwise, from any and all claims, demands, actions or causes of action which in any way arise from the participation and the participation of the minor in the above noted event.
All minors must be at least eight (8) years old. All hikers agree to follow the instructions of the hike leader and stay together as a group. Masks required if in close proximity to other hikers who are not from your household.
I understand that the Sonoma Ecology Center, and all officers, directors, employees, agents, and volunteers of the organization(s), assume no responsibility for damages to personal property.
By my signature, I hereby state that I understand the risks involved in participating in the Day Hike at Saddle Mountain; and willingly and voluntarily accept these risks. By my signature, I hereby surrender and right to seek reimbursement from Sonoma Ecology Center, and its directors, officers, employees, volunteers, and other agents for injury sustained, to me and/or to my child, and liability incurred during my participation in the activity described above. By my signature, I warrant that I am not relying on any oral representations or statements of inducement apart from the statements made on this form.
